ENROLLMENT DOCUMENTS

Dear Pet Parents,

Below are the enrollment documents:

​

• Pet Questionnaire: Please download, fill out and return to us by email or facsimile.
   

• Service Agreement: Please download, fill out and return to us by email or facsimile.
   

• Vaccination Requirements: Please ask your vet to send us your pet's vaccination record and the results of the last fecal test by email or facsimile.  These medical records must come directly from your vet.

Once the enrollment documents are complete, we will invite your dog to come for a play date and join the fun! This will give us a chance to get to know your dog and he/she will meet many new friends. This day is typically 8:00 AM to 4:00 PM and at no charge to you. Please feel free to send us an email with any questions you may have.
